All their lives, from kindergarten to masters’ theses to their first engineering jobs, they’ve gotten ahead by doing things really well the first time. They are often totally unaccustomed to testing and revising.

The test-revise cycle is useful, but I have found that there is no such thing as perfect software, and there's no such thing as software that is "done". We just keep improving it with every iteration.
