Yes, buy from a very small cabal (abt 35 or so) of companies (i.e. what we used to call a Trust!) or pay a minimum tax of $695 (or 2.5% of income, whichever is greater) per family member.
Expect a wave of consolidations among major medical carriers until the number gets to 7 or so, when implicit collaboration starts kicking in and insurance companies start lobbying themselves greater protections from market forces, then watch the number of insurance carriers consolidate down to 3 or 4, like mobile carriers.

no. small cabals are easier to extract protection money from.
